Job summary

Monarch Healthcare in North Carolina is seeking an Assertive Community Treatment Team Substance Abuse Professional II to support individuals with dual mental health and substance use needs. You will coordinate services, develop plans with a multidisciplinary team, and help people live independently in the community.

The role requires a Master’s in Human Services, LCAS-A or LCAS plus licensure, and at least one year of relevant experience.

Qualifications

• Masters in Human Services required.
• Experience in ACT teams is beneficial.

Responsibilities

• Provide service coordination for individuals with mental health and substance use issues.
• Develop and implement the overall service plan with the team.
• Write and monitor Person-Centered Plans and treatment goals.
• Coordinate with program staff, inpatient facilities, and community resources.
• Facilitate independent living skills and community integration.
• Maintain documentation per policies and programs.
