Job summary

Monarch Healthcare in Charlotte, NC seeks an Assertive Community Treatment Team Substance Abuse Professional II to support individuals with mental illness and substance use disorders in developing independence and community integration.

You will coordinate services, build natural supports, and contribute to person-centered plans, collaborating with a multidisciplinary team; after-hours liaison and travel may be required.

Qualifications

• Master’s in Human Services required.
• LCAS-A or LCAS certification required.
• Minimum one year of full-time or two years of part-time applicable experience in the field.

Responsibilities

• Provide service coordination for individuals with mental health and substance use issues.
• Develop natural support networks and provide family-oriented supports.
• Write, implement, evaluate, and revise Person Centered Plans and treatment goals.
• Work as part of a multi-disciplinary team to set and pursue treatment objectives.
• Facilitate independent living skills training and connect clients to community resources.
• Coordinate services with program staff, facilities, detox, and residential programs.
• Review progress and provide ongoing consultation with team and partner agencies.
• Serve as after-hours liaison and communicate regularly with staff.
• Attend meetings and maintain certifications; comply with agency policies and regulations.
• Complete intake assessments covering medical, psychiatric, educational, and vocational history.
• Document activities per service records requirements and write event notes.
• Travel may be required.
