Job summary

Monarch Healthcare in North Carolina is seeking an Assertive Community Treatment Team Substance Abuse Professional II to help clients develop skills and independence while navigating mental health and substance use challenges. You will work within a multidisciplinary team to support daily living, rehabilitation, and community integration.

The role requires LCAS/LCAS-A or QP certification and a Master’s in Human Services with at least one year full-time or two years part-time applicable

Qualifications

• Masters in Human Services required.
• Valid Driver's License (USA) and LCAS/LCAS-A or QP certifications.
• Experience with mental health or substance use disorders preferred (1–4 years).

Responsibilities

• Provide service coordination for individuals with mental health and substance use issues and develop overall service plans.
• Assist in developing natural support networks and provide counseling and linking to community services.
• Write and maintain person-centered plans and treatment goals based on assessments.
• Coordinate with MDTs and ensure ongoing care and after-hours communication.
