Job summary

Bon Secours in Richmond, VA, through St. Mary’s Palliative Care, seeks an Advanced Social Worker to provide high-level clinical social work, assess needs, develop care plans, and counsel patients and families. The role collaborates with a multidisciplinary team to support holistic well-being.

Requirements include an MSW, LSW license in the practice state, and preference for LCSW/LISW. Two years in social/human services are expected.

Qualifications

• MSW required for practice in state(s).
• LSW license in state of practice required.
• Licensure as LCSW or LISW preferred.

Responsibilities

• Evaluates patient needs, develops individualized care plans and advanced intervention strategies.
• Provides emotional support, education and counseling to help patients cope with challenges.
• Manages cases, tracks progress, and refers to community resources.
• Advocates for patients and addresses systemic barriers.
• Responds to crisis situations and connects patients to resources.
• Coordinates care with a multidisciplinary team.
