Job summary

Mercy Health seeks an Advanced Social Worker at St. Charles Hospital to provide direct patient care under supervision, including psychosocial assessments and discharge planning.

You will administer PHQ-9, C-SSRS, and SDOH screenings and collaborate with care teams in ambulatory and hospital settings. The role requires a MSW or MS in Social Administration, state Social Work licensure, and 2 years’ healthcare experience; LISW/LCSW preferred.

Qualifications

• Master of Social Work or Master of Science in Social Administration required.
• Licensed as a Social Worker in the state of practice required.
• LISW or LCSW preferred.
• Minimum 2 years of healthcare, social services, or human services experience.

Responsibilities

• Provide individual, family, and group psychotherapy in ambulatory and hospital outpatient settings.
• Conduct psychosocial assessments, PASRR completion, discharge planning, and post-discharge calls.
• Understand guardianships, advanced directives, and power of attorney (POA).
• Administer PHQ-9, C-SSRS, and Social Determinants of Health screenings.
• Offer community resource knowledge and collaborate with care teams.
• Participate in continued education and in-service training.
