Qualifications

• MSW or MS in Social Administration required.
• Licensed as a Social Worker in state of practice (required).
• Two years of healthcare or social services experience (required).

Responsibilities

• Provides Individual, Family, & Group Psychotherapy in ambulatory and hospital outpatient settings.
• Conducts psychosocial assessments and PASRR completion.
• Performs discharge planning and post-discharge follow-up calls.
• Assists with guardianships, advanced directives, and POA processes.
• Administers PHQ-9, C-SSRS, and Social Determinants of Health screenings.
• Participates in continued education and in-service training.
