Job summary

Project FIND is seeking a Service Coordinator to provide case management and support for older adults living at the Woodstock Hotel in New York City. The role involves conducting assessments, developing service plans, and coordinating care with various departments and external parties.

The ideal candidate should have a Bachelor's degree or relevant experience, and strong interpersonal skills, preferably bilingual in Spanish. This position offers a collaborative working environment focused on the well-being of our clients.

Qualifications

• Bachelor's degree or minimum of 2 years relevant experience strongly preferred.
• Experience working with older adult populations and families preferred.
• Ability to communicate effectively with diverse individuals.

Responsibilities

• Conduct intake and assessment screenings for client needs.
• Implement interventions through counseling and advocacy.
• Work closely with various departments for comprehensive tenant support.
• Facilitate groups on harm reduction and wellness topics.
