Job summary

WelbeHealth is seeking a Social Work Manager to lead the Social Work team and oversee social services and behavioral health programs for seniors staying home and in the community.

The role requires managing staff, ensuring assessments and interventions meet care plans, and coordinating with behavioral health professionals. A social work license is preferred, along with experience with frail or elderly populations and strong communication skills.

Qualifications

• Licensure in social work preferred.
• Minimum of one year of experience with frail/elderly population.
• Excellent clinical, organizational, and communication skills.
• Valid driver’s license with clean record; vehicle insurance evidence.
• CPR and First Aid certification within 180 days of hire; preferred at hire.

Responsibilities

• Manage Social Work Supervisor, Social Workers, and Behavioral Health Specialists, and supervise related staff.
• Oversee effectiveness of social work assessments and IDT participation.
• Ensure delivery of social work interventions per care plans and urgent needs.
• Ensure compliance in documentation, care plan completion, reviews, and rights/grievance processes.
• Oversee Behavioral Health collaboration for groups, counseling, and family education.
• Participate in end-of-life care coordination and home visits as needed.
