Job summary

WelbeHealth seeks a Social Worker reporting to the Social Work Manager to manage a caseload of frail senior participants, conducting psychosocial assessments, care planning, counseling, and case management in coordination with the IDT. You will partner with primary care physicians, facilitate care conferences, arrange resources, support transitions, and contribute to discharge planning.

Qualifications

• Master’s degree in social work (MSW) required.
• Minimum of one (1) year of experience working with a frail or elderly population preferred.
• Experience coordinating and facilitating care conference meetings preferred.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Responsibilities

• Conduct social work assessments to determine psychosocial needs, preferences, and goals of participants and actively participate in IDT meetings to develop care plans.
• Deliver and document social work interventions, including arranging resources and services, assisting with care transitions, providing individual and group counseling, and case management.
• Partner with the Primary Care Physician and other IDT members to guide smooth care transitions between settings (e.g., hospitals, SNFs, home).
• Initiate, coordinate, and facilitate care conference meetings to ensure high levels of care coordination among team members and participants’ networks.
• Provide discharge planning when participants disenroll from the program.
