Job summary

Salgado-LLC is seeking a Social Work Manager to lead our geriatric social work team and guide the delivery of high-quality social services and behavioral health programs. Responsibilities include mentoring staff, ensuring high-quality assessments, coordinating interventions, and maintaining compliance across social work activities, with home visits as needed.

Qualifications

• MSW degree required.
• Active state driver’s license with clean DMV record.
• Proof of recent vehicle insurance.
• Minimum 1 year of experience working with a frail or elderly population.
• Excellent clinical, organizational, and communication skills with patients, families, and team members.

• Maintain compliance across social work functions, including documentation, assessments, care planning, reviews, and patient rights and grievance processes.
• Collaborate with Behavioral Health Specialists to design and oversee support groups, individual counseling, family education, and community partnerships.
• Support patients and families through end-of-life care planning and coordination.
• Conduct home visits as needed to support patients’ ongoing care.
