Job summary

PACS Group is seeking a Social Worker to support residents in a healthcare setting. You will collaborate with families to plan and implement services that promote independence and wellbeing, and coordinate with multidisciplinary teams to address social needs.

Ideal candidates hold a Bachelor’s in Social Work or Human Services, with MSW preferred, and bring strong communication, empathy, and organizational skills to assist with care planning and community resource referrals.

Qualifications

• Bachelor's degree in Social Work or Human Services is required.
• MSW is preferred to enhance clinical practice.
• Experience in health care social work is advantageous.
• Strong communication and interpersonal skills are essential.

Responsibilities

• Assist residents in achieving the highest practicable level of self-care, independence and well-being.
• Provide social services to support physical, mental and psychosocial wellbeing.
• Develop care plans for social services issues and coordinate with the care team.
• Counsel residents and families, offering emotional support and coping resources.
• Coordinate with community resources and social service agencies to meet resident needs.
• Document resident social service status and assist with discharge planning.
