SNF Healthcare is seeking a Social Services Director to oversee the Social Services Department, guiding its development, implementation, and ongoing evaluation to support residents’ well-being while ensuring compliance with regulations.

The role includes coordinating preadmission services, discharge planning, community resource access, and accurate documentation in residents' medical records, with a focus on ethical social work practices.

Responsibilities

• Oversee the development, implementation, supervision and ongoing evaluation of the Social Services Department.
• Identify the need for medically-related social services and ensure compliance with State and Federal regulations.
• Direct the social services component of the comprehensive assessment and contribute to care plan goals and approaches.
• Coordinate preadmission services, participate in interdisciplinary evaluation for institutional care, and complete PASARR Level I.
• Facilitate residents’ transitions back to the community through discharge planning and community-based arrangements.
• Assist residents and representatives in locating financial, legal, and other community resources.
• Ensure documentation of social services actions in medical records and uphold professional standards in records.
• Oversee the establishment and modification of departmental QA procedures.
