Job summary

NHS Management, LLC is seeking a qualified Social Services Director to oversee the social services department in a skilled nursing facility, ensuring residents' emotional and social needs are met in line with federal, state, and local standards.

The role requires a Bachelor’s in Social Work or related field, appropriate licensure, and healthcare experience, with strong coordination across medical staff, residents’ families, and community agencies to develop and implement care plans.

Qualifications

• Bachelor's degree in Social Work or related field (e.g., sociology, psychology, rehabilitation counseling).
• Must meet federal and state certification/licensure requirements for the facility.
• Healthcare experience preferred.
• Ability to interact tactfully with residents, families, staff, and public.
• Must be at least 18 years old and able to communicate in English.
• Must be capable of performing the essential job functions and meet physical/sensory requirements.

Responsibilities

• Plan, develop, organize, implement, evaluate, supervise and direct the social services programs and activities.
• Ensure an ongoing quality assurance program with corrective plans.
• Implement social service interventions to achieve treatment goals and link supports to enhance quality of life.
• Coordinate with administration, medical and nursing services and other departments.
• Ensure forms, reports and progress notes are properly completed.
• Participate in community planning, care plan development, and discharge planning.
• Coordinate with residents, families, staff and community agencies.
• Provide information on support agencies and financial assistance programs.
• Maintain incident and accident reports related to behavior management and elopement.
• Maintain open communication with residents, families, staff, and healthcare resources.
