Responsibilities

• Plan, develop, organize, implement, evaluate and direct social service programs.
• Maintain documentation and advise residents/families on social plans and funeral arrangements.
• Coordinate discharge planning and care planning with staff and families.
• Attend or support meetings related to care plans and social services.
