Berlin Health & Rehabilitation Center in Barre, Vermont is seeking a full-time Speech-Language Pathologist (SLP) to join our in-house rehabilitation team. The role focuses on evaluating and treating residents with speech, language, cognitive, and swallowing disorders to improve communication and function.

The ideal candidate holds Vermont licensure and ASHA certification, has SNF experience, and demonstrates strong clinical and collaborative skills in a patient-centered environment.

Qualifications

• Active Vermont Speech-Language Pathology license required.
• ASHA certification in good standing required.
• Experience in SNF or long-term care preferred.

Responsabilités

• Evaluate residents for speech, language, cognitive, communication, and swallowing deficits.
• Develop and implement individualized treatment plans to improve cognition, communication, swallowing, and functional independence.
• Provide assessments and therapeutic interventions per established plans of care.
• Coordinate MBSS and FEES with physicians as indicated.
• Maintain accurate billing records and clinical documentation.
• Educate residents, families, and caregivers; collaborate with the care team.
• Protect resident privacy and comply with regulations.
