RHF is seeking a Social Service Coordinator for Stone Creek Village in Phoenix, AZ. This part-time, non-exempt role involves assessing residents' needs and coordinating services, building health and wellness programs to support well-being and independence for older adults in the community.

Qualifications

• Bachelor’s degree preferred in Gerontology, Human Services, Social Work, Psychology, Sociology, or related field.
• Experience considered in lieu of degree.

Responsibilities

• Assess residents' needs to identify services that can enhance quality of life and promote independence.
• Establish and maintain connections with local service providers for access to social, health, and wellness resources.
• Develop and sustain educational health and wellness programs for residents.
• Track and document referrals, services, and resident interactions.
• Advocate for residents and liaise between residents and community services.
• Build relationships with community agencies and identify new services for residents.
• Communicate effectively with residents and service providers to maintain supportive environments.
• Collaborate with management and other departments to provide social services and trainings.
• Provide crisis information, referrals, and support as needed.
• Maintain up-to-date records and monthly reports.
