Mayo Clinic in Rochester seeks an experienced Registered Nurse in Senior Services to coordinate geriatric care, deliver assessments, and educate patients and families in home and assisted living settings within a 20‑mile radius. Driver’s license and personal vehicle are required with mileage reimbursement.

The RN will lead care planning, patient education, and transitions of care, while contributing to professional practice and quality improvement. TN sponsorship is eligible for this role.

Qualifications

• RN with a bachelor’s degree in nursing from an accredited program; licensure in-state.
• At least 1 year RN experience in applicable care settings.
• Excellent communication and teamwork skills.
• Ability to drive personal vehicle with mileage reimbursement.

Responsibilities

• Coordinate nursing care, direct patient care, education, and transitions of care.
• Provide assessments and education to geriatric patients in home or assisted living within 20 miles of Rochester.
• Lead and support through preceptor roles and quality improvement efforts.
