Job summary

Mayo Clinic in Rochester, MN seeks an RN for Senior Services to deliver care coordination and patient education in home or assisted living settings within 20 miles of Rochester. You will assess, educate, and coordinate nursing care, working with patients and families across the care continuum.

Travel and flexible hours are required; a valid driver’s license and vehicle are needed, with mileage reimbursement.

Qualifications

• Graduate of a baccalaureate nursing program or higher.
• One year RN experience in an applicable care setting or one year of LPN experience at Mayo Clinic.
• Excellent communication skills (verbal and written).
• Computer skills; EMR experience preferred.

Responsibilities

• Coordinate nursing care and patient education.
• Provide assessments and education to patients in their homes or assisted living facilities.
• Triage and coordinate care via phone and in person within 20 miles of Rochester; mileage reimbursed.
• Lead and participate in quality improvement and professional practice activities.
