Job summary

Mayo Clinic in Minnesota seeks an experienced Registered Nurse for Senior Services to coordinate geriatric care and deliver nurse-led triage by phone and in person. You will assess, educate, and support patients in their homes or nearby facilities within 20 miles of Rochester, driving quality, safety and smooth care transitions.

Candidates must have RN licensure, BLS, and a strong teamwork mindset; ability to manage flexible hours and drive to patient sites; prior EMR experience is preferred.

Qualifications

• RN licensure in applicable state
• BLS certification required
• Bachelor’s degree in nursing or higher; CCNE/ NLN accreditation preferred
• Minimum one year RN experience in applicable care setting
• Experience in team environments and strong communication
• Proficient computer/EMR skills; able to work flex hours
• Ability to manage flexible schedule including evenings/nights/weekends
• Demonstrated leadership and critical thinking skills

Responsibilities

• Coordinate nursing care and provide direct patient care and education
• Deliver triage and assessments for geriatric patients via phone and in person
• Coordinate transitions of care and collaborate with care teams
• May drive personal vehicle within 20 miles of Rochester; mileage reimbursed
