Job summary

Silver Tree Residential, LLC is seeking a dependable Service Coordinator to support residents at our senior communities. You will build partnerships with agencies, deliver on-site programs, and coordinate services that promote independence for seniors.

The ideal candidate holds a bachelor’s degree in gerontology or social work, has at least three years in social services, and demonstrates strong communication, organizational skills, and MS Office proficiency.

Qualifications

• Bachelor's degree in gerontology or related field; minimum three years in social services
• Strong written and verbal communication skills; ability to problem-solve, educate and advocate for vulnerable adults
• Proficiency with MS Office and Excel; ability to maintain organized records and reports

Responsibilities

• Develop relationships with agencies and nonprofits to provide services for residents
• Organize educational programming, wellness presentations and on-site events
• Serve as liaison with medical and professional services providers and community agencies
• Report abuse, neglect, or exploitation per state statutes and ethical standards
• Maintain resident files in HUD and STR compliance and prepare HUD reports
• Create and maintain a resource directory for the community
• Assist residents with transitions into and out of the facility as needed
• Support site inspections and coordinate plans with management and residents
• Create monthly calendar and site newsletter with input from staff
• Attend home visits to engage with prospective tenants about the service coordination program
