Qualifications

• At least two years of experience working with older adults.

Responsibilities

• Conduct comprehensive psycho-social assessments of clients’ needs.
• Provide extensive case management on an ongoing basis in clients’ homes.
• Collaborate with service providers and parishes to coordinate client needs.

Bilingual Spanish/English

Strong interview and assessment skills

Excellent time-management skills

Strong verbal and written communication skills

Intermediate Microsoft Word and Excel skills

Education

Bachelor’s Degree in Behavioral Science, Human Services or Social Services

LMSW or LCSW or temporary license
