Job summary

Catholic Charities is seeking a Case Manager III for Aging and Disability Services in Saint Paul. You will assess client needs, develop service plans, coordinate with community resources, and advocate to improve outcomes. Strong organizational and trauma-informed practice are essential.

The role requires in-home assessments, documentation, and collaboration with a team to support clients’ goals and independence.

Qualifications

• Bachelor’s or master’s degree in social work, psychology, sociology, human services, or related field preferred.
• Bachelor’s with a major in any field and at least 1 year of social worker/case manager experience may qualify.

Responsibilities

• Provide initial and annual assessments and develop service plans for clients, including in-home visits.
• Coordinate with Catholic Charities programs and community resources to meet client needs.
• Advocate for clients and maintain complete case documentation as required.
