Job summary

Catholic Charities in Houston seeks a Lead Case Manager to oversee wrap-around services for pregnant women, single parents, and families. You will supervise a team of case managers, ensuring high-quality, client-centered support across healthcare, social services, education, and employment.

You will guide policy implementation, collaborate with counseling services, and facilitate life skills and job training programs to empower clients toward self-sufficiency.

Qualifications

• Associate degree in social work, psychology, or a related field.
• Community Health Worker Certification or obtain within the first 6 months of hire
• Bilingual in English and Spanish
• High School Diploma or GED
• Three (3) years of experience in case management, or a focus on maternal and child health, pregnancy support preferred.
• Valid Texas Driver’s License

Responsibilities

• Provide wrap-around case management to pregnant women, single parents, and families, including intake assessments and ongoing support.
• Lead the program case managers, providing guidance and mentorship for effective service delivery.
• Coordinate care across healthcare, social services, education, and employment to ensure holistic support.
• Conduct home visits and regular check-ins to monitor progress and address barriers to success.
• Collaborate with program leadership to develop policies and best practices.
• Support clients with prenatal and postnatal care, parenting, and family dynamics.
• Coordinate with Counseling program for group sessions and access to mental health services.
• Assist clients with health insurance enrollment, Medicaid, and other financial assistance; connect to housing, food, childcare, and utilities resources.
• Facilitate life skills, budgeting, parenting, and job training workshops.
