Case Manager will provide direct support and case management services to senior residents, promote their health and well-being, and help foster a safe, supportive, and thriving community environment. This position plays a vital role in ensuring residents have access to essentialPurpose of Position: The purpose of the Case Manager position will be to join a team of social workers to provide social services that enhance the clients’ overall quality of life and increase their access to a variety of resources. Our focus is on financial stability, health and wellness, food, and housing security. The ideal candidate will possess strong interpersonal skills to facilitate collaboration with the team and together deliver impactful resultsSchedule: Monday through Friday 35 hours per weekSalary Range: $47,000-$50,000Candidates must have the required qualifications to be considered for the position.
