Job summary

Volunteers of America Texas in Dallas is seeking a social services professional to assess residents’ needs and tailor individualized service plans. You will build relationships with community providers and coordinate referrals for case management, transportation, counseling, and wellness services.

Role requires a background in social work or related field, with experience in senior or disabled populations; a Texas driver license is preferred.

Qualifications

• Bachelor's degree in Social Work or related field preferred.
• High School Diploma or equivalent required; two years' experience in social service delivery with senior citizens and non-elderly disabled preferred.

Responsibilities

• Providing informal, individual assessments of residents to evaluate health, psychological and social needs and developing tailored service plans.
• Establishing relationships with community agencies and setting up a provider directory for staff and residents.
• Referring residents to service providers (case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, visiting nurse, wellness) as needed.
• Educating residents on service availability, procedures, and client rights; providing advocacy as appropriate.
• Monitoring ongoing service provision from community agencies and keeping case management up to date with progress.
• Managing the provision of supportive services where appropriate.
• Helping residents build informal support networks with other residents, family and friends.
