Job summary

VOA-Texas in Dallas is seeking a dedicated professional to join our team. The role focuses on assessing resident needs, coordinating services, and building community connections to support independent living.

You will work with Human Resources to connect residents with case management, transportation, meals, and wellness services while educating them on program options and rights. A bachelor’s in social work or related field is preferred, with driver license and relevant experience;

Qualifications

  • Bachelor's degree in Social Work or related field preferred with one year experience in social service delivery; OR High School Diploma with two years' experience in social service delivery preferred.
  • Must have valid Texas Driver License, clear MVR and ability to be insured.

Responsibilities

  • Providing informal, individual assessments of residents and developing tailored case plans with periodic reassessment.
  • Establishing relationships with community agencies and setting up a directory of providers for staff and residents.
  • Referring residents to service providers in the community (case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, preventative health screening/wellness and legal advocacy).
  • Educating residents on service availability, application procedures, client rights, etc., and providing advocacy as appropriate.
  • Monitoring ongoing provision of services from community agencies and updating progress with case management and providers.
  • Helping residents build informal support networks with other residents, family and friends.

Day-To-Day

  • Providing informal, individual assessments (i.e., evaluation of health, psychological and social needs, development of an individually tailored case plan for services and periodic reassessment of the resident's situation and needs) for a resident when such service is not available through the general community.
  • Establishing new relationships with agencies and service providers in the community and setting up a directory of providers for use by both project staff and residents.
  • Referring residents to service providers in the community (Ex: case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness and legal advocacy).
  • Educating residents on service availability, application procedures, client rights, etc., providing advocacy as appropriate.
  • Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual. Manages the provision of supportive services where appropriate.
  • Helps residents build informal support networks with other residents, family and friends.

Qualifications

  • Bachelors degree in Social Work or related field preferred with one year experience in social service delivery with senior citizens and non-elderly disabled preferred OR High School Diploma or equivalent required and two years' experience in social service delivery with senior citizens and non-elderly disabled preferred.
  • Must have valid Texas Driver License, clear MVR and ability to be insured.
