Job summary

Volunteers of America Texas is seeking a dedicated Wellness Coach to work with residents in the Dallas area. You will deliver individual and group wellness coaching, plan health and social activities, and connect residents with healthcare and community resources.

Qualifications include a Bachelor's degree in a related field and at least 2 years of experience with older adults or community health. Requires a Texas driver’s license and ability to work collaboratively with staff and partners.

Qualifications

• Bachelor's degree in social work, Public Health, Psychology, Human Services, or related field preferred.
• At least 2 years of experience working with older adults or in community-based health or social services.
• Ability to work independently while building collaborative relationships with residents, families, and community partners.

Responsibilities

• Delivers individual and group wellness coaching and education.
• Plans and facilitates health, wellness, and social engagement activities.
• Connects residents with healthcare, behavioral health, and community resources.
• Coordinates referrals and follow up on resident needs and goals.
• Maintains accurate documentation and assist with program reporting.
• Collaborates with Service Coordinators, Community Administrators, and community partners.
