Qualifications

• Bachelor’s degree in Social Work or related field preferred with one year experience in social service delivery with senior citizens and non-elderly disabled preferred.
• High School Diploma or equivalent required and two years’ experience in social service delivery with senior citizens and non-elderly disabled preferred.

Responsibilities

• May provide informal, individual assessments of residents’ health, psychological and social needs and develop tailored service plans.
• Establishes linkages with community agencies and providers; identifies best pricing and flexible services for residents.
• Sets up a directory of providers for use by staff and residents.
• Refers residents to service providers in the general community.
• Case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, and wellness services.
• Educates residents on service availability, procedures, rights and advocacy as appropriate.
• Monitors ongoing provision of services and keeps providers up to date on resident progress.
• Helps residents build informal support networks with family and friends.
