Job summary

Volunteers of America Texas in Dallas is seeking a social services professional to assess residents’ needs and tailor individualized service plans. You will build relationships with community providers and coordinate referrals for case management, transportation, counseling, and wellness services.

Role requires a background in social work or related field, with experience in senior or disabled populations; a Texas driver license is preferred.

Qualifications

• Bachelor's degree in Social Work or related field preferred.
• High School Diploma or equivalent required; two years' experience in social service delivery with senior citizens and non-elderly disabled preferred.

Responsibilities

• Providing informal, individual assessments of residents to evaluate health, psychological and social needs and developing tailored service plans.
• Establishing relationships with community agencies and setting up a provider directory for staff and residents.
• Referring residents to service providers (case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, visiting nurse, wellness) as needed.
• Educating residents on service availability, procedures, and client rights; providing advocacy as appropriate.
• Monitoring ongoing service provision from community agencies and keeping case management up to date with progress.
• Managing the provision of supportive services where appropriate.
• Helping residents build informal support networks with other residents, family and friends.

Day-To-Day:

Providing informal, individual assessments (i.e., evaluation of health, psychological and social needs, development of an individually tailored case plan for services and periodic reassessment of the resident's situation and needs) for a resident when such service is not available through the general community.

Establishing new relationships with agencies and service providers in the community and setting up a directory of providers for use by both project staff and residents.

Referring residents to service providers in the community (Ex: case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness and legal advocacy).

Educating residents on service availability, application procedures, client rights, etc., providing advocacy as appropriate.

Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual.

Manages the provision of supportive services where appropriate.

Helps residents build informal support networks with other residents, family and friends.

Qualifications

Bachelors degree in Social Work or related field preferred with one year experience in social service delivery with senior citizens and non-elderly disabled preferred OR High School Diploma or equivalent required and two years' experience in social service delivery with senior citizens and non-elderly disabled preferred.

Must have valid Texas Driver License, clear MVR and ability to be insured.
