A-Line Staffing Solutions in the McAllen, TX area seeks a Senior LTSS Service Care Manager (RN) to support members with complex healthcare needs. This hybrid role combines in-home visits with remote documentation, paying $46–$48 per hour on a 6-month contract basis.

Expect standard 8-to-5 weekdays and frequent travel within the assigned area. The RN will review records, coordinate services, and ensure timely assessments and documentation, while working independently in a field-based environment.

Qualifications

• Active RN license is required.
• Degree: ASN, BSN, or MSN.
• More than three years of nursing experience.
• At least two years in case management.
• Valid driver's license and ability to travel to members' homes.
• Strong clinical judgment and organizational skills, with ability to prioritize duties.

Responsibilities

• Review electronic medical records and prior authorizations before visits.
• Conduct in-home assessments (initial, change-in-condition, MDCP, Level 3 reassessments).
• Evaluate members' clinical needs and long-term services requirements.
• Identify gaps in care and coordinate requested services.
• Complete assessments within 24 hours of visits.
• Submit service initiation requests within three business days and finish other docs within five days.
• Coordinate care transitions to the assigned manager after about 15 business days.
