Qualificações

Responsabilidades

Location: McAllen, Texas area. Pay Rate: $46–$48 per hour. Schedule: Monday–Friday, 8:00 AM–5:00 PM. Assignment Length: 6-month contract. Openings: 2. Work Arrangement: Remote administrative work with required in-home member visits.

Position Overview

We are seeking experienced Registered Nurses (RNs) for a Senior LTSS Service Care Manager position supporting members with complex healthcare and long-term care needs in the McAllen, Texas area. This role combines clinical assessment, care coordination, medical record review, and in-home visits. The RN will evaluate members' healthcare needs, identify necessary services, and ensure assessments and documentation are completed accurately and within established deadlines. This position is ideal for an RN with case management experience, strong clinical judgment, and the ability to work independently in a field-based environment.

Key Responsibilities

• Review electronic medical records, diagnoses, medications, recent authorizations, and existing services before member visits.
• Conduct in-home assessments, including initial assessments, change-in-condition assessments, MDCP assessments, and Level 3 reassessments.
• Evaluate members' clinical conditions, functional abilities, and long-term services and supports needs.
• Identify gaps in care and coordinate requests for appropriate services.
• Complete and submit assessments within 24 hours of the visit.
• Submit requests to initiate services within three business days and complete other documentation within five business days.
• Coordinate care transitions and transfer members to their assigned permanent care manager or service coordinator after approximately 15 business days.
• Complete required training and administrative assignments within established deadlines.
• Maintain accurate clinical documentation and follow established care management procedures.

Qualifications

• Active Registered Nurse (RN) license.
• Associate Degree in Nursing (ASN), Bachelor of Science in Nursing (BSN), or Master of Science in Nursing (MSN).
• More than three years of nursing experience.
• At least two years of case management experience.
• Valid driver's license and ability to travel to members' homes.
• Strong clinical judgment, organizational skills, and ability to prioritize competing responsibilities.
• Ability to work independently while following established procedures and direction.

Preferred Qualifications

• Pediatric nursing experience.
• Experience with Medicaid, managed care, long-term services and supports (LTSS), home health, or complex care coordination.
• Experience conducting clinical assessments and coordinating services for members with disabilities or ongoing healthcare needs.

Schedule and Travel Requirements

Monday–Friday, 8:00 AM–5:00 PM.

Remote work for documentation and administrative responsibilities.

In-person visits to members' homes in the McAllen, Texas area.

Home visits typically last 2–4 hours.

Travel may require up to two hours each way.

Occasional overtime or Saturday work may be required. Candidates must be comfortable with regular local travel and managing assessment documentation within strict deadlines.

Compensation and Contract Details
