21st Century Home Health Services (21HHS) is seeking a Registered Nurse Case Manager, Level 2, to lead patient care in Sebastopol and surrounding areas. You will oversee 2 Nurses, conduct SOCs, evaluations, and discharge planning, and collaborate with physicians to optimize home health outcomes.

In this leadership role, you will develop care plans, educate patients and families, document thoroughly, and support a patient-centered, high-quality home health program in Northern California.

Qualifications

• CA RN license required.
• BLS/CPR certification required.
• Experience in home health case management and leadership preferred.

Responsibilities

• Oversee 2 Nurses who implement the care plan.
• Perform Start of Care, Evaluations, Resumption of Care, and discharge planning.
