21st Century Home Health Services (21HHS) in California seeks an experienced Registered Nurse (RN) Case Manager, Level 2, to lead a small team and coordinate patient care in the home.

Anticipated territory includes Yuba City and surrounding areas; you will conduct SOCs, evaluations, and discharge planning, develop advanced care plans, and mentor two Nurses while collaborating with physicians and the interdisciplinary team to prevent hospital readmissions and ensure high-quality outcomes.

Qualifications

• Minimum one year of home health case management experience.
• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience supervising nurses is highly preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is required.
• Case management fundamentals are required.

Responsabilités

• Create, implement and execute the overall plan of care to promote wellness and prevent re-hospitalization.
• Provide oversight of two Nurses responsible for executing the care plan.
• Conduct SOCs, Evaluations, Resumption of Care and discharge planning.
• Develop advanced care plans and mentor patients’ families/caregivers.
• Collaborate with physicians and agency staff to deliver high-quality care.
