Job summary

21st Century Home Health Services Inc. is seeking a Registered Nurse Home Health Case Manager Level 2 to assess patients in their homes and develop advanced care plans. You will collaborate with an interdisciplinary team to drive improved outcomes and manage care episodes.

Required are CA RN licensure and BLS/CPR, with 1+ year Home Health Case Management experience. The role offers autonomy, flexible scheduling, and a focus on patient-centered care in the Bay Area.

Qualifications

• One year of Home Health Case Management experience preferred.
• Minimum one year of nursing experience required; two+ years preferred.
• Experience supervising/mentoring nurses preferred.
• Current and unrestricted CA RN license.
• Current BLS/CPR certification.
• Valid CA Driver's License.
• High level of clinical knowledge in home health care.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is required.
• Case Management fundamentals are required.

Responsibilities

• Perform a full assessment of the patient and family during initial visit.
• Develop an advanced plan of care including rehabilitation.
• Work with patients and family one on one to achieve improvement and/or stability.
• Case manage patient care episodes with interdisciplinary clinical team.
• Prepare clinical notes, orders and charting in a compliant time frame.
• Collaborate with physicians and agency staff to deliver the best care possible.
