Job summary

21st Century Home Health Services Inc. is seeking a Full Time Registered Nurse Case Manager, Level 2, to cover Sebastopol and surrounding areas. The RN will lead two nurses, develop and implement care plans, and coordinate with physicians and an interdisciplinary team to prevent rehospitalizations.

Qualified candidates will hold a CA RN license, BLS/CPR, and have Home Health Case Management experience. A valid CA driver's license is required with reliable transportation to patient homes.

Qualifications

One year of Home Health Case Management experience required.
Minimum of one year of Nursing experience required; two+ years preferred.
Current and unrestricted CA RN license; BLS/CPR certification.
Valid CA Driver's License and reliable transportation.

Responsibilities

Create, implement and manage the overall plan of care to promote wellness and prevent re-hospitalization.
Oversee two Nurses in the care plan execution.
Conduct SOCs, Evaluations, Resumption of Care and discharge planning.
Collaborate with physicians and interdisciplinary team to deliver quality care.
Provide education to patients and families in the home setting.
