Job summary

21st Century Home Health Services (21HHS) is hiring an RN Case Manager, Level 2, to lead two nurses and oversee care plans for patients in Citrus Heights and surrounding areas. You will perform SOCs, evaluations and discharge planning, ensuring high-quality home health oversight and collaboration with physicians.

This role requires CA RN licensure, BLS/CPR, and a proactive, empathetic leadership style to manage complex cases in patients’ homes while upholding regulatory standards and delivering

Qualifications

• One Year of Home Health Case Management experience preferred.
• Minimum of one year of Nursing Experience required; two+ years preferred.
• Experience supervising, mentoring Nurses is highly preferred.
• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• High level of clinical knowledge in home health care is key.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is required.
• Case Management fundamentals is required.

Responsibilities

• Create, implement and execute the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
• Oversee 2 Nurses responsible for executing your care plan.
• Conduct comprehensive patient assessments such as Start of Cares (SOCs), Evaluations, Resumption of Care and discharge planning.
• Collaborate with physicians and agency staff to deliver the best care possible.
• Prepare clinical notes, orders and charting in a compliant time frame.
