Responsibilities

• Performs concurrent patient assessment related to level of care and expedites the plan of care.
• Maintains competency in InterQual criteria.
• Collaborates with social work to address discharge planning.
• Documents and provides discharge planning services to optimize post-hospital care needs.
• Collaborates with the interdisciplinary team to transition the patient to the next level of care.
• Applies evidence-based standards to evaluate care, length of stay, and resources.
• Develops relationships with community organizations to facilitate appropriate care.
• Collaborates with staff and post-discharge providers to ensure safe discharge.
• Facilitates care conferences with patient/family and healthcare team.
• Supports service-excellence initiatives and quality care strategies.
• Acts as expert resource to team and participates in improvement activities.
• Assists with orientation and mentorship for new case managers.
