Description:The position involves managing a caseload of 13-25 patients.The focus will be on both Utilization Review and discharge planning.Candidates should expect to work with complex patients, including those on ventilators, with airways, IV antibiotics, and chronic versus acute illnesses.Tasks include collaborating with patients, families, and a multidisciplinary team to ensure patient progression through the continuum of care.Responsibilities include independently developing a plan of care for each assigned patient from admission through discharge.The role requires managing throughput and length of stay, ensuring appropriate resources are used efficiently and cost-effectively.Interdisciplinary rounding and care coordination are essential parts of the job.Family meetings, utilization review, insurance authorization, concurrent reviews, advanced directives, and discharge planning are also key duties.The position involves referrals for durable medical equipment, home health versus outpatient services, and transportation resources.Team conferences will occur Monday through Friday for rehabilitation, with interdisciplinary rounding on designated days.Strong analytical and problem-solving skills are critical.Proficiency in Microsoft software is expected.Excellent interpersonal, verbal, and written communication skills are necessary for interacting with diverse populations.The ability to multitask and prioritize in a fast-paced environment is essential.A basic understanding of acute and post-acute levels of care and resources is needed.Holiday and weekend work requirements are limited, with flexibility regarding weekend calls.Scrubs or business casual attire is acceptable for the role.
