Job summary

Jobtome in New Mexico is seeking a dedicated Nurse Case Manager to oversee patient triage, assignment to care managers, and coordination of multidisciplinary services. The role emphasizes quality, cost-effective patient care within a collaborative team.

You will supervise staff, act as a resource to the health care team, and ensure adherence to policies while coordinating resources and discharge planning for varied patient ages.

Qualifications

• Bachelor's Degree required.
• 3 years directly related experience.
• RN in NM or reciprocal agreement.
• CPR for Healthcare/BLS within 30 days.
• Bilingual English/Keres, Tewa, Tiwa, Towa, Zuni or Navajo is a plus.

Responsibilities

• Review and triage patients for assignment to case managers.
• Coordinate multidisciplinary services for organized patient care.
• Supervise staff in the department and act as resource to staff.
• Ensure adherence to hospital policies and continuity of care.
• Assist in discharge planning and resource coordination.
