Qualifications

• Bachelor's Degree is required.
• RN license in NM or via reciprocal agreement.
• 3 years directly related experience.
• CPR for Healthcare/BLS required within 30 days.
• Bilingual English and additional languages is a plus.

Responsibilities

• Review, triage and assign patients to case managers.
• Coordinate systems and services for organized, multidisciplinary care.
• Supervise staff in the department.
• Act as a resource and role model for staff.
• Ensure adherence to hospital policies and procedures.
• Develop collaborative relationships with departments and community agencies.
• Oversee case loads and staff hours.
• Conduct timely discharge planning.
• Maintain documentation for patient progress.
