Job summary

Presbyterian Healthcare Services in New Mexico seeks an RN Case Manager to provide clinically-based case management across the continuum of care. The role supports length of stay optimization, discharge planning, and safe transitions with physicians and care teams.

Qualifications

RN licensure in New Mexico expected.
5 years of utilization review or case management experience required; or a Master's in Nursing.
National Case Management certification preferred.

Responsibilities

Manage a designated caseload and coordinate discharge planning.
Apply clinical criteria for admissions and continued stay determinations.
Lead discharge planning and arrange post-acute services to prevent delays.
Collaborate with physicians, nursing, social work, and ancillary services.
Document accurately and support performance improvement initiatives.
