Job summary

Jobtome is seeking a Registered Nurse - Case Management Supervisor in New Mexico to coordinate patient care across disciplines, review admissions and assign caseloads. You will supervise staff, ensure adherence to care standards, and serve as a primary contact for patients, families, and care teams.

The role emphasizes patient-centered planning, discharge coordination, and collaboration with physicians, RN, and community resources; prior case management experience is valued.

Qualifications

• Essential: Bachelor's Degree; Master’s Degree preferred.
• Experience: 3 years directly related experience.
• Credentials: RN in NM or eligible per NM rules; CPR/BLS certification.

Responsibilities

• Coordinate initial review, triage and assignment of patients to case managers.
• Supervise staff in Care Management Services and act as resource for the team.
• Assist in discharge planning and coordination with physicians, RN staff and community resources.
• Identify patient needs and facilitate access to financial resources and support services.
• Oversee and optimize patient care processes and documentation.
