Centene is seeking a Senior Care Manager RN to assess, plan, and implement complex care management activities for members with various health needs. The role involves developing personalized care plans, educating members and caregivers, and coordinating with providers to ensure quality, cost-effective care.

The position supports Medicaid/STAR CHIP populations with a standard schedule Monday–Friday, 8:00 a.m.–5:00 p.m.

Qualifikationen

• Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 4 - 6 years of related experience.
• RN license required.

Aufgaben

• Develops and continuously assesses ongoing care plans and collaborates with providers to address member needs.
• Coordinates between member/family and care provider team to ensure person-centered care.
• Monitors progress toward care plan goals and revises plans as needed.
• Identifies problems/barriers for care management and initiates interventions.
• Reviews member data to identify trends and improve quality of care.
• Reviews referrals and intake assessments to develop appropriate care plans.
