Job summary

Qualifications

• RN care coordination experience in adult populations (21+) in Home Health, Hospice, Long-Term Care, or Managed Care is preferred.
• Bachelor of Nursing degree preferred.
• RN licensure required; state or compact licensure.

Responsibilities

• Assess and coordinate medical and support services for complex/high acuity members.
• Develop and monitor personalized care/service plans for long-term care members.
• Educate members and families on services, benefits, and care options.
