Job summary

Wellstar Health System seeks a Care Coordinator RN Sr to assess transitional care needs and coordinate care across the continuum during acute admission. The role partners with physicians and care teams to optimize treatment plans, streamline discharge, and plan for post-discharge care.

The RN Sr serves as a resource for the team, mentors new hires, and participates in interdisciplinary rounds to ensure efficient care progression and alignment with patient goals.

Qualifications

• Associates degree in nursing or nursing diploma; Bachelors preferred.
• Minimum 3 years in acute care nursing.
• Minimum 2 years in hospital or payer-based case management.
• RN license (state) and BLS/ARC certifications required.
• Excellent written and verbal communication skills.
• Ability to collaborate with physicians and care teams.

Responsibilities

• Assess transitional care needs and coordinate care across the continuum.
• Engage with patient and family to ensure care needs are met.
• Lead discharge planning and post-discharge coordination.
• Mentor and precept new hires as needed.
