Hybrid role with home visits and remote work; Mon-Fri 8:00 AM–5:00 PM CST. Requires nursing degree and RN licensure; RUG certification within 90 days preferred. Travel within Brownsville, Harlingen and San Benito is required.

Qualifications

• 4–6 years of RN care coordination or case management experience.

Responsibilities

• Assess and coordinate medical and supportive services for complex/high acuity members.
• Develop and update long-term care/service plans.
• Educate members and families on services and benefits.
• Collaborate with care teams to ensure person-centered care.
