Project NOW, Inc. seeks an Outreach Supervisor to coordinate services and lead outreach staff in Illinois. The role includes assessing client needs, making referrals, and following up to ensure outcomes are met.

You will supervise outreach workers, maintain confidential client data, and assist in planning activities sponsored by the Senior Center, while ensuring high-quality customer service.

Qualifications

• Associate's degree or equivalent and 6 months to 1 year related experience or training.

Responsabilités

• Coordinate and follow-up with resources to ensure services are delivered.
• Assist in long-range planning by identifying gaps and overlaps in services.
• Interview and assess needs, provide information, referrals, and follow-up.
• Provide information about community resources meeting expressed needs.
• Place individuals with appropriate resources or providers.
• Conduct follow-up to confirm service delivery.
• Complete required reporting forms.
• Attend training and seminars as directed.
• Supervise Outreach Workers daily.
• Maintain confidential information and provide statistics when required.
• Assist in planning advocacy ranges and Senior Center activities.
