Job summary

Telligen is seeking a Community Liaison / Resource Navigator in Oak Brook, IL to connect individuals with community resources and provide non-clinical support to care managers. You will develop partnerships with community resources, facilities, state agencies, and community entities to improve access to housing and support services.

The Illinois Tailored Care Management Program helps individuals with complex health and social needs transition to community living.

Qualifications

High School diploma or equivalent with 3 years of experience in community-based services.
Bachelor’s degree in social work or related field.
Certified care and/or case manager.
Licensed Practical Nurse (LPN) or health coach with at least one year of experience.
Community Health Worker certification through college level program or ability to obtain certification.
In-depth knowledge of social determinants of health and how they impact healthcare.
Valid Illinois driver’s license and reliable transportation.

Responsibilities

Based on the member’s person-centered service plan, make referrals for transition services in alignment with the plan.
Provide follow up and monitoring of resource referrals and coordinate activities within timelines.
Guide members to appropriate member benefit information.
Develop and maintain a resource library for members in Cook County and across the state as needed.
Collaborate with care managers to coordinate healthcare and transition services throughout the continuum of care.
Escalate issues through established channels between clinical and non-clinical staff.
Collaborate with all disciplines to promote enrollee compliance and use of resources.
Assist members and families with personal and environmental resources to obtain benefits from medical care.
Perform miscellaneous duties as assigned.
