Job summary

Medica is seeking a Remote Care Coordinator in Minnesota to support members with individualized care plans and coordinate delivery of medical and social services. Our Care Coordinators work from home with a flexible 4- or 5-day schedule, no evenings, weekends, or holidays, and mileage reimbursement for community visits within the service area.

The position offers a salary range of $72,100 to $97,850, plus a comprehensive benefits package and opportunities to collaborate with physicians,

Qualifications

• Active, unrestricted RN license in MN required
• MnCHOICES Certified Assessor Training (MNCAT) preferred or ability to obtain within 21 days
• Public Health Certification preferred, however not required
• Experience in home and community based services or with seniors
• Ability to travel within Medica's service area

Responsibilities

• Manage a specified caseload and coordinate delivery of services
• Educate members about Medicare and Medicaid benefits
• Collaborate with physicians, providers and county entities on care plans
• Perform other duties as assigned
