Job summary

Centene Corporation is recruiting for a remote Care Management Liaison to support the IL YouthCare program. The role involves member outreach via phone or home visits, coordinating care plans with providers, and answering member inquiries while ensuring continuity of care.

The position requires a Bachelor’s degree in nursing, social sciences, or related field for IL Youth Care, with 2–4 years of related experience.

Qualifications

• Requires a High School diploma or GED.
• Requires 2 - 4 years of related experience.
• For Illinois Youth Care plan: Bachelor’s degree in nursing, social sciences, social work, or related field; one year of supervised clinical experience in human-services; must reside in IL.

Responsibilities

• Provides outreach to members via phone or home visits to engage members and discuss care plan/service plan.
• Coordinates care activities with providers and partners to accommodate changes or progress.
• Addresses inquiries regarding care plans and services.
• Develops knowledge of care management services and handles escalated issues.
• Communicates with care managers and practitioners to facilitate member services and continuity of care.
• Performs service assessments/screenings for members with complex needs and documents care needs.
• Maintains member records per state/regulatory requirements and shares as needed.
• Trains and supports team as needed.
• Performs other duties as assigned.
